Heath Ledger just doen't strike me as crazy enough to play the Joker. Jack Nicholson was outta his mind and that was really brought out with that character. Johnny Deep was the guy I thought would make for a great Joker, rumors for awhile were that he was in the running for the character, as well as Adrian Brody.

I thought they did a great job with everything in Batman Begins... except on one certain scene:

It was the silly-as-fuck scene where he is driving his multi-ton car/vehicle/tank over city roof-tops. Afterwords, the whole movie went downhill for me. I couldn't help it. The movie would have had all my respect if it wasn't for that small lapse in realism. Was it a necessary scene? Nope, the vehicle was amazing enough already.

I was reminded of that scene in Batman Forever, where Batman hooked his car to a building and drives up the side of the building to avoid a gigantic inferno. I don't remember how he landed it but needless to say, he didn't crash and burn to death in jet-fuel. Maybe someone should tell the makers of the Batman franchises: Batman is just a dude with a suit and bunch'a gadgets. Damn. If I want cheesy craptastic action, I'll watch a movie starring Jason Statham. Other than that scene, I loved Batman Begins.

Everything you mentioned is explained in detail in the movie. I just think that the movie-makers should quit while they're ahead. They've already wowed us with a Batman story that is supposed to be serious and "dark". Why go further and make it just plain silly?
