I just saw a presentation tonight by photographer Nick Vedros. It was incredible to to hear his stories about his carrier and successful business in commercial photography. He is based out of Kansas City and he works with a lot of big name clients. He spoke for about 2 hours going over a slew of his shots. The best part was when he explained how he made each shot and even showed shots of the sets and lighting. He was using HMI lots on a few of his sets. He said they are the Hollywood lights and has the full intensity of the sun at just 7 feet away.
Check out his site.

I was drawn to the cereal bowl one too. He did show it in the presentation and admitted that the army guys were CGI. I think he said this shot was for Army Men the video game or something like that. It was quite interesting to here how he coordinated with the CGI guy so they would have the same lighting. They figured since it's cereal and cereal is eaten in the morning the light should be low and backlit to replicate the sun.
Shots like these and other client shots can sell for upwards of $50000 and he still has full rights to the image. He says when he has clients like Captial One, Walmart, etc that it's pocket change to them.
